As a core pillar within Population Health, preventative care includes a variety of resources and support. This ensures the best possible quality of life regardless of a person’s current state of health. In the words of Lindsey Sand, Vice President of Population Health at Knute Nelson, “Population Health is a division that aims to complement, enhance, and introduce new care models and concepts into our continuum.”
Focusing on Prevention with Population Health
By encouraging optimal well-being through a blend of upstream approaches, Population Health seeks to get ahead of the health curve. Along the way, we leverage proactive partnerships. This includes healthcare and community-based organizations, health plans, and LeadingAge MN Foundation to eliminate siloed care.
The collaboration of these practices, procedures, and technologies allows us to break down barriers standing in the way of prevention. This might be regular screenings, social isolation support, medical transportation, or one of the many options we offer. Our one-on-one level of detail helps us understand and address individual needs on the path to preventative care.
Best said by Lindsey, “Preventative care and well-being support can include assisting with a regular primary care routine, coordinating for social needs, and wellness education and programs.” Through our wide umbrella of services and support offerings, we ensure seamless access even throughout rural areas.
Maintaining a Proactive Stance
Our proactive stance toward prevention means making a broad bundle of services available to communities far and wide. To best uphold our mission of enriching the lives of everyone we serve, we recognize the value of tailoring these services on an individual basis. In Lindsey’s experience, “the approach from there becomes unique to the client.”
For example, while some may benefit from PEARLS (Program to Encourage Active, Rewarding Lives) to mitigate depression through a series of counseling sessions, others may benefit from a blend of physical, mental, or spiritual wellness resources.
Navigating the Path to Preventative Care
We believe in getting to know those we serve like family. That’s why our preventative care services begin with a personalized meeting with one of our care managers or specialists. From there, we ensure mutual understanding surrounding your goals before establishing connections to the most relevant resources and support as part of our process.
Through our collaborative approach, access is only the first step. Our integrated system means seamless communication with all members of an individual’s care team including family members, supportive service providers, and physicians.
This multidisciplinary approach helps further our commitment to whole-person wellness. In addition to keeping key preventative considerations from falling through the cracks. As the Vice President of Population Health at Knute Nelson, Lindsey Sand is a dynamic force in the realm of senior care. With a strong commitment to her profession, Lindsey has dedicated her career to serving aging adults. Her deep connection to her grandparents and elder neighbors growing up, as well as frustrations with the healthcare system, guided her path toward industry disruption.
She holds licenses as a nursing home administrator and health services executive. As well as a B.A. in Psychology and Human Services and a graduate certificate in Long-Term Care Administration. Sand has the expertise to drive positive change. She plays a vital role in various organizations including the Lakes Area Age Friendly and LeadingAge Minnesota. This includes the Chair for the LeadingAge Minnesota Foundation. Beyond her professional life, Lindsey shares her life with her husband, Brian, and their two spirited daughters, Lucy and Lily. The family’s hobbies include basketball, cheerleading, gymnastics, church life, and family time. Sand’s journey is an inspiring testament to the commitment to the well-being of older adults.
